There are several resources available to UM faculty, staff and students who wish to create an online survey:
Qualtrics
U-M offers all its faculty, students, and staff free access to Qualtrics - an excellent tool for creating and distributing online surveys. There are a few conditions of that access, that you should be aware of:
- It is to be used only for surveys that you create in your role as SPH faculty, student, or staff, and not for anything outside of that role, e.g. your own business, summer internships, jobs outside of SPH, volunteer organizations, etc.
- Make sure to obtain IRB approval for any surveys that require such approval. Access to this tool does not diminish the importance of rules governing collecting and protecting personal information in research.
- Faculty, staff and students can set up their own accounts by simply logging into Qualtrics with their uniqname and Kerberos password.
- There are no group accounts in Qualtrics. Each individual needs to have an account created. There are collaboration groups in Qualtrics, however. Once everyone has an account, you can ask Patty or Vlad to create your group. It is easy to share surveys with everyone in the group.
- Qualtrics support can be obtained from ITS by calling 4-HELP (4-4357) or emailing 4help@umich.edu. You can also use the Qualtrics online help system.
- Patty Bradley (sph.web@umich.edu) can adjust account permissions as needed.
Other Survey Options
- Google Forms make it easy to record poll/survey data in a spreadsheet and can be used for online surveys.
- Canvas is available to faculty and staff for creating online tests and quizzes for course sites. Work is now in progress to migrate all project sites away from CTools. View the ITS Project Overview.
- Survey Gizmo is a non-UM tool to create and publish custom surveys that are accessible to persons with disabilities.
- Survey Monkey is a non-UM tool to create and publish custom surveys.
