IC Requesting Hardware from Hardware Lifecycle

Summary

Procedure and information required when placing a request from the Hardware Lifecycle Team (HLT) for pc replacement or a new hardware request. 

Requests are made using the IC Computer Request Form if you are making a request without a ticket, if there is a ticket add information and transfer ticket.

Submitting Hardware Requests

Requests for hardware are made using the IC Computer Request Form . This form is intended to be used only by members of LSA Technology Services, please do not provide this link to users, as there is a different Survey sent through mailing lists for users to complete that has fewer options and more detailed information about the choices available. 

This form has several required fields - these are necessary for the completion of HLT tasks, so please make sure to complete them fully and accurately.

Delivery Process

HLT will receive a delivery notification from the business office. If it's a pallet IC will need to assist with retrieving shipment. If it's a small order IC/ HLT will retrieve packages. 

Inventory should receive notice of delivery and IC should coordinate with Inventory with asset management of devices.
