OFA - Request for Waiver of the Noncustodial (Household B) CSS Profile

Students may request a waiver of their noncustodial (HHB) parent's information under certain limited circumstances. Applicants are encouraged to submit their waiver request prior to their admittance to the University of Michigan.

We must receive this form by the following due dates:
• December 1 for students applying for Early Decision
• March 15 for all other students starting in the spring, summer, or fall terms
• November 15 for students starting in the winter term

A. Include a copy of the death certificate or other supporting documentation.
B. Include detailed statements from 1) you, the student; 2) your custodial parent; and 3) an unrelated third-party individual* explaining the special circumstances as they pertain to your personal (not financial) relationship with the noncustodial parent. The statements should outline attempts made to contact the other parent.
C. Attach any supplemental documentation showing the safety concerns (such as police reports), if available.

*The third-party individual must be someone knowledgeable about the family situation (such as a teacher, counselor, clergy member, or an employer of the student’s custodial parent). This person cannot be a relative or close family friend.
