In connection with this request, please be sure to enclose any applicable document(s), including but not limited to:

A certified copy of the Letters of Authority of the personal representative
A copy of the death certificate of the deceased
A small-estate affidavit
A court order

In addition, State law requires the following documents to be attached to the submission of this form:
An original or copy of the power of attorney granting the agent the authority over the content of electronic communications of the principle
An affidavit from the agent under section 5505 of the estates and protected individuals code.

By submitting this request, you understand that:

The digital custodian is not required to give you any information that the user had deleted before the individual had passed.

The digital custodian is not required to give you all the information if it is related to University business. 

The digital custodian may request a reasonable administrative charge for the cost of disclosing digital assets

The digital custodian has 56 days after receipt of the information to comply with a request.
