I certify the following to be true:
- I have requested observation and/or participation at one or more University of Michigan Children’s Centers for the sole purpose of completing a course-related or research project.
- Any data collected will be used for the course-related or research project it was requested for and will then be destroyed.
- All data collected will be coded in such a way that it cannot be linked to a child’s identity.
- Children’s identities, personal information and related data will be kept confidential at all times during completion of the project and will be destroyed when they are no longer needed.
- No photo, video or audio recordings may be taken at any time without the express, written permission of the research and training coordinator and the child’s parent/guardian.
