How do I get new evaluation templates created?
To create a new evaluation template or make changes to an existing template, complete an RO - Create or Update a Template form.
When requesting a new template, you will be expected to provide a name for the template and be specific about which question(s) should appear on the template. We recommend carefully browsing our question catalog when selecting questions for your template. If you are sure that a question does not exist in the catalog, you may request it on this form. For brand new questions, you will be expected to provide the exact phrasing of the question, the question scale, and the question type.
It is not recommended that every section have their own template.
