Evaluation Structure Overview

Department Administrators are responsible for ordering teaching evaluations. One component of ordering evaluations is selecting a "template." This template comprises questions that your department wants to appear alongside the eight university-wide questions. Up to 12 departmental questions are allowed on end-of-term evaluation templates. There is no limit for midterm evaluation templates. Prior to the evaluation opening to students, instructors will have an opportunity to add up to five personalized questions. These would appear alongside the university-wide and departmental questions.

Prior to ordering an evaluation, review your current templates. If your current templates are insufficient, use this form to request the following changes:

Request a New Template

When requesting a new template, you will be expected to provide a name for the template and be specific about which question(s) should appear on the template.  We recommend carefully browsing our question catalog when selecting questions for your template. If you are sure that a question does not exist in the catalog, you may request it on this form. For brand new questions, you will be expected to provide the exact phrasing of the question, the question scale, and the question type. 

It is not recommended that every section have their own template.

Add Questions to an Existing Template

When requesting changes to an existing template, provide the name of the template exactly how it appears in the Google sheet. You may then proceed to request the addition of questions. We recommend carefully browsing our question catalog when selecting questions for your template. To include a question that already appears in the catalog, provide the exact wording and question number. If you are sure that a question does not already exist in the catalog, you may request it on this form. For brand new questions, you will be expected to provide the exact phrasing of the question, the question scale, and the question type. 

Remove Questions from an Existing Template

When requesting changes to an existing template, provide the name of the template exactly how it appears in the Google sheet. To request the deletion of a question from your template, provide the exact wording and question number.

Additional Information

  • All requests must be submitted by the department administrator. 
  • Template changes may take up to one week to process.
