Do not use this form for general equipment checkout.
This form is NOT for general room reservations. Only submit this form if you don't have access to a space, need a long reservation, recurring reservation, or complex reservation. Authorized users can book rooms normally with the SMTD Scheduling System.
Request repair service for equipment in the Tech Suite or checkout equipment.
PAT Faculty and Staff may use this to request Tech Suite access for any Mcard holder. Note that students will still need to complete any required trainings for each space. These trainings are also recommended for faculty/staff.
