How can department staff obtain Grade Change Report Forms?
The Supplemental Grade Report (SGR) forms were discontinued in winter 2007.
Department staff can request Grade Change Reports for exceptions when entering the grade online is not possible by contacting the Records and Enrollment Department at 734-764-6280. Grade Change Reports will only be given to University of Michigan staff with photo identification.
