COVID-19 Vaccination and Childhood Immunization Requirement

Do I need to have a health check or immunizations before attending U-M?


All U-M students residing in Michigan Housing on the Ann Arbor campus are required to complete the U-M Vaccination History Form showing whether or not they are vaccinated for the below vaccinations in line with the CDC immunization schedule. All U-M students living off campus are encouraged to submit their records as well. This information will be shared with University Health Service for treatment purposes.

U-M students who are not vaccinated with any of the below six (6) vaccinations must still complete this form, which will include an attestation that states that they are not vaccinated and understand they may be subject to additional public health protocols should there be a public health need.

  • COVID-19 vaccine
  • Measles, mumps, and rubella (MMR) vaccine
  • Meningococcal(MenACWY) vaccine
  • Polio vaccine
  • Tetanus, diphtheria, and pertussis (Tdap) vaccine
  • Varicella vaccine (chicken pox)

You may want to arrange for a physical exam with your home healthcare provider and update immunizations at that time, but a physical exam is not required by the university. (Athletic teams or clubs may have different requirements.)
